Certified Birth Certificate (within the last 30 days only) to verify identity & name

(Do not obtain this until after you receive BOTH background checks back)

Valid Alabama Driver’s License or State I.D. (not expired) Must have lived here for 90 days or more

Ever been married and changed your name before:  Any and ALL marriage certificates certified (no Xerox copies).

If Divorced - Most Recent Divorce Decree with Seal

Proofs of Russell County Residency

(need two from list -must be in petitioner’s name):

 

•        Voter Registration

•        Current Utility Bill – No Telephone Bills OR Cable Bills

•        Current Year Property Tax

•        Current Year Tag Registration

•        Current Year Medical Billing

PETITION

Certified Birth Certificate (issued within the last 30 days only) to verify identity & name

(Do not obtain this until after you receive BOTH background checks back IF REQUIRED)

If father is listed on the birth certificate, both the mother and father MUST be present for the name change. If either party is not able to be present, a court hearing would be required and you would need to contact an attorney for the name change.

Minor's 14 - 18 must be present for the name change

IF PARENTS ARE MARRIED:

Valid Alabama Driver’s License or State I.D. (not expired) OF BOTH PARENTS

IF PARENTS ARE NOT MARRIED:

Valid Alabama Driver’s License or State I.D. (not expired) OF CUSTODIAL PARENT

Valid State Driver’s License or State I.D. (not expired) OF NONCUSTODIAL PARENT

IF FATHER’S NAME IS NOT ON THE BIRTH CERTIFICATE:

Valid Alabama Driver’s License or State I.D. (not expired) of MOTHER

IF either party have been married and changed their name(s) before: Any and ALL marriage certificates certified (no Xerox copies).

School Aged Minors must present proof of school enrollment in a Russell County School

Temporary guardianship does not permit you to change a child's name.  If you are not the parent and have sole custody of a child and you are trying to change their name, the paperwork must state that the parental rights of the parents were terminated otherwise you will have to have the parents' consent to change the name.
